Procare Health Limited invites applications for the Clinical Lead (Primary Care) Haslemere Proactive Care Hub. This is a local, operational leadership role with a strong primary care perspective, based at Haslemere Community Hospital.
You will work closely with the Clinical Director for Proactive Care Services and the multidisciplinary team to ensure proactive, person-centred care for frail and complex patients.
Clinical Lead (Primary Care) Haslemere Proactive Care Hub 1 to 2 sessions per week
This is an opportunity for an experienced GP who would like to provide local clinical leadership within the Hub while remaining closely connected to general practice.
Working alongside the Clinical Director for Proactive Care Services, Hub operational leadership and the wider multidisciplinary team, you will provide a strong primary care voice within the service.
This is deliberately a local, operational clinical leadership role focused on the Haslemere Hub. It does not carry the wider strategic, financial or system responsibilities of the Clinical Director role.
We would particularly welcome interest from GPs who enjoy collaborative working, supporting colleagues and bringing a strong primary care perspective to integrated services.
Why join us?
This is an opportunity to be part of a developing model of care that brings primary care and community services together around some of our most complex patients.
You will have the opportunity to work closely with experienced clinicians from across primary, community and specialist care, influence how the Hub develops and help demonstrate what genuinely integrated, proactive care can achieve for patients.
Both roles are based primarily at Haslemere Community Hospital.
For an informal conversation about opportunity, please contact Jane Fagan, Chief Operating Officer.
