Procare is seeking a Lead GP for the Haslemere Hub Proactive Care project. The role focuses on local clinical leadership and maintaining close ties with general practice, working alongside the Clinical Director and multidisciplinary teams.

The post-holder will balance leadership duties with direct clinical work, supporting proactive, person-centred care for patients with frailty and long-term conditions, within Haslemere Community Hospital.

Qualifications

• GMC Registered Medical Practitioner
• Demonstrable leadership experience

Responsabilités

• Provide operational GP leadership for the Haslemere Proactive Care Hub.
• Support high-quality multidisciplinary team decision-making.
• Act as the clinical link between the Hub and local GP practices.
• Support the development of the Integrated Neighbourhood Team with partners.
• Ensure safe, evidence-based and person-centred care.
