Job summary

Procare Health Limited invites applications for the Clinical Lead (Primary Care) Haslemere Proactive Care Hub. This is a local, operational leadership role with a strong primary care perspective, based at Haslemere Community Hospital.

You will work closely with the Clinical Director for Proactive Care Services and the multidisciplinary team to ensure proactive, person-centred care for frail and complex patients.

Qualifications

• Essential: GMC Registered Medical Practitioner.
• Desirable: Demonstrable leadership experience.
• Essential: Significant GP Experience.

Responsibilities

• Provide operational GP leadership for the Haslemere Proactive Care Hub.
• Support multidisciplinary decision-making.
• Act as clinical link between the Hub and local GP practices.
• Support development of Integrated Neighbourhood Team with partners.
• Support safe, evidence-based, person-centred care.
• Contribute to service improvement and evaluation.
• Support delivery of clinical outcomes and KPIs.
