Job summary

weareprocare is seeking a GP with an Extended Role to join the Haslemere Proactive Care Hub. The role focuses on proactive long-term condition management and learning to work across traditional boundaries within a multidisciplinary team at Haslemere Community Hospital.

The successful candidate will assess frailty and complex needs, review GP decisions, optimise medications, and contribute to anticipatory care planning. An interest in frailty and proactive community care is essential.

Qualifications

• GMC registration or eligibility to practice in the UK.
• Interest or experience in frailty, proactive care or complex multimorbidity.
• Experience working in an MDT or community-based care setting.

Responsibilities

• Conduct comprehensive assessments of patients with frailty and complex needs.
• Provide GP clinical review and interpretation of specialist recommendations.
• Support medication optimisation and develop personalised anticipatory care plans.
• Collaborate with MDT members across primary, community and specialist care.

Job description

We are looking for experienced and enthusiastic GPs to join the Haslemere Proactive Care Hub, working as part of a multidisciplinary team supporting people living with frailty, multiple long-term conditions and complex care needs.

The Hub brings together primary care, community services and specialist expertise to provide more proactive, coordinated care closer to home.

Our aim is simple: to identify people who would benefit from additional support earlier, bring the right professionals around them and help prevent deterioration and avoidable hospital admission.

Main duties of the job

GP with Extended Role Proactive Long Term Conditions Hub 4-6 sessions per week

We are also looking for GPs who have a particular interest in frailty, complex multimorbidity, long-term conditions and proactive community care.

This is predominantly a clinical role, working directly with patients and alongside the wider Hub multidisciplinary team.

You will undertake comprehensive assessment of people with frailty and complex needs, including Comprehensive Geriatric Assessment (CGA); provide GP clinical review and sense-checking of specialist recommendations; support medication optimisation; develop personalised care and anticipatory care plans; and work alongside specialist clinicians, pharmacists, therapists, community nursing, social care, mental health and other members of the MDT.

A particularly important part of the role will be bringing the generalist GP perspective to complex care looking beyond individual conditions and ensuring that recommendations are practical, coordinated and appropriate for the person as a whole.

You do not need to be a specialist geriatrician. We are looking for experienced GPs with an interest in developing this area of practice and enthusiasm for working differently across traditional organisational boundaries
