Job summary

Responsibilities

• Provide active consultant leadership and oversight to the Community Adult Learning Disability Service on the Isle of Wight.
• Work with senior clinical team to ensure clear team objectives and safety.
• Ensure effective communication within and outside the team for timely information sharing.
• Coordinate care across teams and manage patient information sharing and contact points.
• Support risk identification and management within the team's area of responsibility.
