Northumbria Healthcare NHS Foundation Trust invites applications for a Community Matron to join our Community Nursing Teams across North Northumberland. You will identify vulnerable patients at risk, coordinate proactive case management with a multi-disciplinary team, and provide care home support to improve outcomes and reduce hospital admissions.

- Undertakes advanced clinical assessment, identifies needs, plans, coordinates care for complex patients.
- Provides clinical leadership within Community Nursing and supports staff development and competence assessment.
