An Intermediate Care Support Assistant position is available within Brookfield Support Centre, which provides bed-based intermediate care in the community. We help people avoid hospital admissions, promote independence after hospital stays, and delay residential home placements until truly necessary.
This role is part of a multidisciplinary team working with therapists, nurses, social care, and medical practitioners to promote independent living and support service users' return to the community. You will assist in rehabilitation and care programs designed to encourage independence and self-management.

Applicants should have experience caring for people, supporting personal care, emotional, and social needs with kindness, respect, and dignity. Knowledge of safeguarding vulnerable adults is desirable, but training will be provided. Effective communication with team members is essential. Experience may be from work or lived experience.
The role involves working on a 4-week rolling rota with early (7.30 am - 3 pm) and late shifts (2.30 pm - 10 pm), including weekends and bank holidays, with additional allowances. We offer an excellent pension scheme and opportunities for professional growth.
