Making a difference as an Intermediate Care Support Worker in Contact Cares Brookfield Centre


An Intermediate Care Support Assistant post has become available within Brookfield Support Centre, which provides bed based intermediate care in the community. We help people avoid going into hospital, be as independent as possible after a stay in hospital and prevent people from having to move into a residential home until they really need to.

This post is part of a multi-disciplinary team, working together with therapists, nurses, social care and medical practitioners to promote independent living, support and plan their return back into the community. You will work with our service users to assist in their rehabilitation and care programmes, which have been developed by our multi-disciplinary team to encourage independence and self-management.

You will have experience of caring for people and be able to provide support with personal care, emotional and social needs in a kind, caring, trustworthy and respectful manner, that preserves dignity and self-respect at all times. You will have some knowledge of supporting service user needs and safeguarding of vulnerable adults, though training will be given, and be able to clearly and confidently communicate with team members. This experience can be lived experience and/or work experience.
You will work to a 4 week rolling rota early (7.30am 3pm inclusive of 30 mins unpaid break) and late ( 2.30pm-10pm inclusive of 30 mins unpaid break) which will include weekend and bank holiday working, for which an additional allowance will be paid . There is an excellent pension scheme and a commitment to your professional development.
