Job summary

North London NHS Foundation Trust (NLNFT) is recruiting four Band 6 Community Mental Health Practitioners to join the newly established Intensive Outreach Service. You will work as a Care Coordinator, managing defined caseloads and delivering recovery-focused interventions in the community.

Qualifications

• Registered Mental Health Practitioner with current professional registration.
• Experience in community mental health services with adults.
• Ability to work in a multidisciplinary team.

Responsibilities

• Act as Care Coordinator for a defined caseload of adults with complex mental health needs.
• Undertake comprehensive biopsychosocial, risk, mental capacity and safeguarding assessments.
• Develop, implement and review care plans and crisis plans.
• Provide intensive outreach in community settings to promote engagement and recovery.
• Coordinate care across multidisciplinary and multi‑agency settings.
