Job summary

Central and North West London NHS Foundation Trust (CNWL) is seeking a Band 6 Occupational Therapist to join a new Assertive Outreach pathway within Hillingdon Adult Community Mental Health Services. The role involves delivering time-limited, occupation-focused interventions and proactive engagement across various community settings, including patients’ homes.

It requires flexible, face-to-face work and collaboration within a multidisciplinary team.

Qualifications

• Degree or Diploma in Occupational Therapy (OT).
• HCPC registered as an OT.
• Post-graduate training relevant to practice.

Responsibilities

• Deliver time-limited, occupationally focused interventions.
• Provide assertive outreach with proactive engagement during risk periods.
• Assess impact of illness on daily functioning and participation.
• Plan and deliver goal‑oriented interventions with clear outcomes.
• Manage referrals and maintain a small, high‑intensity caseload within a MDT.
• Apply OT models to inform clinical reasoning and care planning.
