Job summary

Hoop Social Work in Bridgend is seeking an experienced Adults Social Worker to join the Integrated Network Team on an initial 3 month contract. The role is full-time with hybrid working (2 days at home, 3 on site) and a pay rate up to £35.32 per hour.

You will support adults 18+ with frailty and long-term conditions, working in a multi-disciplinary team to assess, plan and coordinate care with regular on-site visits.

Qualifications

• Qualified Social Worker with post-qualifying adults experience.
• Registered with Social Care Wales.
• Driver with access to a car and willing to drive.

Responsibilities

• Adults community social work within an Integrated Network Team.
• Managing complex cases with a strengths-based, person-centred approach.
• Care coordination and multi-agency working.
• Regular visits required (driver essential).
