Job summary

Hoop Social Work is recruiting an experienced Social Worker to join Swansea Council's Community Mental Health Team (CMHT) in Adult Services for an initial 3-month contract. This role offers a balance of office-based, home-working and community-based practice with hybrid working.

You will assess needs, develop care plans, coordinate with health colleagues, and uphold safeguarding while working within statutory frameworks. Salary rates: £38.43 per hour Umbrella or £30.50 per hour PAYE.

Qualifications

• UK social worker registration and current registration with Social Care Wales or Social Work England.
• Enhanced DBS (Update Service) or willingness to apply.
• Minimum 6 months' UK experience as a registered Social Worker.
• 3 years' referencing history and full driving licence with access to a car.

Responsibilities

• Assess needs, strengths and risks of adults, carers and families and identify support.
• Act as Care Coordinator, developing and reviewing person-centred care plans.
• Support individuals to maintain independence and positive outcomes in the community.
• Assess and manage risk, including safeguarding and protection of vulnerable adults.
• Coordinate with health professionals and partner agencies to package care.
• Ensure individuals' views and rights remain central to planning and decision-making.
• Maintain case records and complete statutory social work tasks.
• Work under the Social Services and Well-being (Wales) Act 2014 and Mental Health Act 1983.
• Manage caseload with supervision from Team Leader/Senior Practitioner.
