A community health organization in Liversedge is seeking an Integrated Neighbourhood Team (INT) Lead to develop and deliver integrated community care. The successful candidate will lead a team to provide high-quality services through collaboration with various health and social care professionals. We are seeking an enthusiastic, motivated and reliable leader who is keen to support the development and delivery of Integrated Neighbourhood Teams (INT) in partnership with providers within Kirklees.
You will be required to have the energy, enthusiasm and drive to support the team to deliver a high-quality service, as well as excellent professional and communications skills.
The successful candidate will work with internal colleagues and partner providers in order to ensure all elements of the INT Team Program is delivered within operational services. Working in partnership with other providers is essential for this role.
INTs will support delivery of holistic personalised and care in the community, which is focused on co-ordinating care across different services for patients to best meet their health & Wellbeing needs. This involves developing a more personalised, proactive, and preventative approach to care in a community setting.
INTs generally comprise a group of professionals from across, social care, community health care, mental health services, primary care and local voluntary and community sector organisations who come together to deliver care for people within their area.
