Job summary

Sirona care & health, England’s largest provider of Adult’s and Children’s Community Services, seeks an experienced Band 6 Case Manager to join the Integrated Network Team in South Gloucestershire. You will undertake holistic assessments, coordinate care after hospital discharge, and work with GPs, social care, and care homes to support people with complex needs in the community.

Job description

We are looking for an experienced and motivated clinician to join our Integrated Network Team (INT) in South Gloucestershire as Band 6 Case Manager.

This is an exciting opportunity to work at the heart of a truly integrated service, bringing together health, social care and community partners to support people with complex needs following hospital discharge.

Working as part of a multi-disciplinary team, you will undertake holistic assessments, develop person-centered management plans and coordinate care for individuals with complex health and social care needs.

You will work closely with GPs, social care teams, acute hospitals, care homes, voluntary organisations and other system partners to ensure seamless, integrated support for patients across the community.

This role requires excellent clinical reasoning, strong organisational skills and the ability to work autonomously while contributing positively to team leadership and service development.

• Contribute to service improvement, quality initiatives and audit activities.
• Support and develop colleagues, students and junior team members.
• Assist with team coordination, workload allocation and capacity management.
