Cheshire and Wirral Partnership provides CHC nursing services on Wirral. We are seeking a Registered Nurse to manage CHC assessments, care commissioning and case management for individuals eligible for NHS funded care.

You will work across the Wirral with occasional visits further afield, requiring a driving licence and access to a vehicle. The role emphasizes collaboration with hospitals, social services, families and carers, with strong focus on KPIs, data input, and safeguarding.

Qualifications

  • Educated to degree level or equivalent in a specialist area.
  • Post qualifying and continuing professional development (CPD).
  • Current NMC Registered Nurse.
  • Knowledge of Social Services and Local Authority functions.
  • Understanding of CHC/NHS Funded Care guidance for adults.
  • Strong NHS operational and clinical understanding.
  • Knowledge of patient experience and quality.
  • Familiar with commissioning agendas and primary care.

Responsibilities

  • Undertake CHC assessments and ensure alignment with the National Framework and submitted KPIs.
  • Complete all review assessments in line with KPIs.
  • Collaborate with acute hospitals to support discharge to assess pathways.
  • Input information onto departmental databases adhering to NMC record-keeping standards.
  • Assist individuals and representatives in CHC processes.
  • Commission care packages for CHC or NHS funded care as per policy.
  • Case management and ongoing reviews for CHC caseloads.
  • Raise quality/safeguarding concerns through the reporting routes promptly.

Job description

We have an exciting opportunity for an individual (Registered Nurse) to join a successful and established Continuing Healthcare (CHC) team on the Wirral. The team is responsible for providing commissioning services for people with complex care needs who are eligible for NHS funded care., Full details are available on the JD / Person Specification but in summary the successful candidate will need to :

  • Ensure assessments are undertaken in line with the National Framework for Continuing Healthcare and funded-Nursing Care and submitted to the ICB within the service's key performance indicators
  • Complete all review assessments in line with the service's key performance indicators
  • Work closely with acute hospitals to support discharge to assess pathways
  • Accurately input information onto departmental databases and tools used in Continuing Healthcare adhering to NMC standards for record keeping
  • Assist and support individuals and representatives in relation to the Continuing Healthcare process
  • Commission appropriate packages of care for individuals eligible for CHC or other NHS funded care, in line with the Commissioning Policy.
  • Case management and ongoing review of individuals eligible for CHC or other NHS funded care in accordance with the National Framework for Continuing Healthcare and NHS-Funded Nursing Care
  • Provide assurance regarding quality of individual packages of care by undertaking reviews as required and reporting quality/safeguarding concerns through the agreed reporting route in a timely manner;

The post offers superb opportunities to utilise your assessment and case management skills whilst learning within a supportive, experienced and successful team. The candidate should have experience of undertaking CHC assessments, working with patients, families and carers, and have a sound knowledge of the NHS Framework for Continuing Healthcare. The successful candidate will be responsible for the process from referral to assessment, review, care commissioning and case management for individuals eligible for CHC You will need to be an Registered Nurse with current registration and be able to work independently and as part of a team, be comfortable using initiative and knowing when to ask for guidance. In return you'll have access to all necessary support, administration help and clinical supervision. This role ordinarily involves travelling across the Wirral with occasional visits further afield therefore a valid driving licence and access to a vehicle for work purposes is essential.

Qualifications & Experience
  • Educated to degree level or equivalent level of experience of working in specialist area.
  • Evidence of post qualifying and continuing professional development.
  • Current NMC Registration., Full understanding of Social Services and other Local Authority functions
  • Conversant with national and local guidance on continuing care/NHS Funded Care for adults.
  • Strong understanding of the operational management and clinical role of the NHS.
  • Understanding of the patient experience agenda & its contribution to quality.
  • Knowledge & understanding of the current NHS agenda and of primary care., Understanding of the commissioning agenda including its scope, structure, methods of operation, processes and inter-relationships, Track record in clinical quality & performance activities.
  • Within a Community setting.
  • Experience of partnership working with other agencies.
