Job summary

Caritas Recruitment Limited is seeking an experienced Adult Social Worker to join a locality team. You will work with adults with diverse needs to promote independence, wellbeing and choice, delivering safe, personalised support.

You will complete strengths-based assessments, review care plans, coordinate services, manage risk and safeguard where needed, while building relationships with families, carers and partner agencies.

Qualifications

• Must hold a recognised Social Work qualification and be registered.
• Experience with adults in statutory or community settings desirable.
• Knowledge of strengths-based practice, safeguarding, mental capacity, best-interest decision-making.

Responsibilities

• Conduct strengths-based assessments with adults and review care plans.
• Identify eligible needs and arrange services per local procedures.
• Develop outcome-focused plans with individuals, families and carers.
• Manage risk and safeguard adults when concerns arise.
• Contribute to multi-disciplinary meetings and coordinate support packages.
• Make referrals and maintain accurate, timely records.
• Prioritise a varied caseload and make sound decisions in complex situations.
