North Northamptonshire Council is seeking a compassionate and proactive individual to join the newly established Access Team within Adult Social Care. You will manage a varied caseload, undertake strengths-based assessments and coordinate services to help adults live independently.

The role focuses on developing person-centred support plans, safeguarding, and collaboration with families, carers and partner organisations. A driving licence is required and hybrid working arrangements are in place.

Qualificações

• Applicants must hold a valid driving licence and have access to a vehicle for work purposes.
• Excellent organisational and communication skills are essential.
• Experience in adult social care or safeguarding is desirable but not required.

Responsabilidades

• Manage a varied caseload and undertake strengths-based assessments.
• Develop and review support plans and coordinate services.
