Job summary

Sohl Associates Limited is seeking a dedicated Care and Support practitioner in Mansfield to help service users live independently and safely. You will help express preferences, implement tailored care plans with risk assessments, and support daily living tasks including meals, personal care and medications.

The role emphasises safeguarding, accurate record-keeping, and coordination of care using IT systems, with shift patterns that may include weekends and nights.

Qualifications

• Ability to devise and implement individual care and/or support plans with risk assessments.
• Experience in safeguarding adults and/or children in care settings.
• Maintain service user records and document progress of plans.

Responsibilities

• Support service users/tenants to express themselves and make informed choices about their lives.
• Devise and implement care plans with risk assessments.
• Assist with medications and personal care as needed.
• Carry out domestic tasks, such as cooking and cleaning, as part of support.
• Follow and review plans and record progress of care.
• Safeguard service users and respond to safeguarding concerns.
• Maintain health and safety records and use IT systems for documentation.
• Work on a rota including weekends and night duties if required.
