Job summary

Chimes in Exton, PA is seeking a Supported Living Specialist to help consumers reach self-sufficiency through individualized interventions within a therapeutic milieu. You will work on a full-time schedule, Monday through Friday, 9:00 am – 5:00 pm, with flexible scheduling as needed.

Qualifications include an Associate's Degree and 2 years of mental health direct service or case management experience; CPRP certification is a plus.

Responsibilities

Provide services per program description and standards.
Assist the program coordinator with administrative duties.
Monitor outcomes using Quality of Life Inventory and satisfaction surveys; adjust as needed.
Develop and maintain service plans and coordinate with adjunct providers.
Identify resources to support clients and encourage utilization.
Intervene appropriately in crisis situations.
Deliver hands-on support to build skills outlined in service plans.
Maintain accurate records of billable services and submit timely.
