Job summary

NHS Scotland is seeking an experienced Health and Social Care Worker to join the Integrated Community & Support Services. You will work with the Physiotherapy Service to deliver treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital.

You will hold your own caseload and assess, progress, and discharge patients autonomously within set parameters, with a focus on older people who comprise the main user group.

Qualifications

• HNC in Health and Social Care or SCQF level 8 (e.g., Postural Stability Instructor)
• Experience working in the NHS as a therapy support worker
• Experience working with older people as they are the main service users

Responsibilities

• Deliver therapeutic treatment programmes to service users in their own homes
• Hold and manage a personal caseload, assessing, progressing and discharging within parameters
• Work with the Physiotherapy Service within Intermediate Care to support rehabilitation and prevent hospital admission
