Lincolnshire Community Health Services NHS Trust is seeking a Promoting Independence Therapy Assistant to support patients in Grantham after hospital discharge. The role focuses on rehabilitation and reablement to help people live independently at home.

You will work with a multidisciplinary team, assess needs, plan simple interventions, and assist with equipment and home adaptations, across flexible hours seven days a week.

Competenze

• 5 GCSEs at grade 4 or above, or equivalent.
• A Level 3 Vocational Healthcare Qualification or Equivalent.
• Moving and handling training/experience.

Mansioni

• Assist in assessment and rehabilitation of individuals post discharge.
• Provide reablement care to patients in their homes.
• Work with the multi-disciplinary team to promote independence.
• Carry out environmental assessment within the patient’s home.
• Communicate effectively with patients, carers and health professionals.
• Collaborate with other agencies to determine long-term care needs.
• Deliver complex care as prescribed and plan simple interventions.
• Ensure safe use of all equipment within scope.
