Job summary

Buckinghamshire Healthcare NHS Trust is seeking a Palliative Care Community Nurse to deliver compassionate end-of-life care in patients' homes across Buckinghamshire. This role focuses on symptom management, rapid assessment, and coordinated care with GP practices and district nursing teams.

You will develop personalised care plans, provide emotional support to families, and contribute to a virtual ward when appropriate, ensuring safe, patient-centred care every day.

Responsibilities

• Deliver safe, effective, compassionate personalised care.
• Administration of end-of-life subcutaneous PRN medications (PAR & medications to be obtained via GP.
• Work closely withGPs, district nurses, social services, and family caregivers to ensure seamless support.
• Provide Emotional & Psychological support to patients & families.
• Request for face-to-face review of unstable/deteriorating patients from Hub CNS, or OOH consultant. The patient is then to be placed on virtual ward.
• Develop personalised care plans
• Verification of Death.
