Job summary

St Michaels Hospice in Hereford seeks a Registered Nurse to deliver high quality palliative and end of life care in patients' homes or care homes. Shift-based role with unsocial hours payments and a responsibility to lead and support a multi-disciplinary team.

You will assess, plan and provide compassionate care while adhering to NMC guidelines and safety policies. Driving is required for home visits; training and DBS checks are included as part of ongoing development.

Qualifications

• Live Registered Nurse with broad knowledge of general nursing care
• Knowledge of national and local health initiatives
• Keyboard basic IT functions; full UK driving licence (over 21) and held for 12 months

Responsibilities

• Care Provision: conduct holistic assessment, plan and deliver nursing care, report changes to the team, supervise and upskill Senior HCAs, provide expert telephone advice to professionals, patients and families.
• Communication: update patients and families, facilitate multidisciplinary teamwork, capture patient preferences for care planning, handle complaints per policy.
• Professional Leadership: provide clinical supervision and mentorship, deliver training, deputise for senior nurses.
• Quality, Improvement, Audit and Research: participate in audits and reflect on care quality, ensure incidents are recorded and safety policies followed.
• Education: attend mandatory training, maintain professional portfolio and infection control practices.
