Job summary

Sheffield City Council invites applications for an Inclusion officer (Young Carers Assessor) on a 12-month fixed-term basis. You will work directly with children and young people with caring responsibilities, placing the young person’s voice at the centre of assessments.

You will build trusted relationships with families and partner agencies, planning support with schools, health, social care and voluntary partners to improve outcomes for young carers across the city.

Qualifications

• Experience working with children, young people or families.
• Understanding challenges faced by young carers.
• Skilled in assessment, engagement and person-centred planning.
• Ability to build positive relationships with children, families and partner agencies.
• Commitment to improving outcomes and reducing inequalities.
• Excellent communication, organisational and recording skills.
• Ability to work independently while contributing to a team.

Responsibilities

• Identify needs, strengths and aspirations of young carers through assessments.
• Place the young person’s voice at the centre of all decisions.
• Build trusted relationships with young carers and families to access the right support.
• Collaborate with schools, health professionals, social care and voluntary sector partners.
• Plan support for the cared-for person and for the young carer, including transitions.
