Job summary

Oxford Health NHS Foundation Trust is seeking an experienced Occupational Therapist to join the Oxfordshire Health and Homelessness Inclusion Team (OHHIT). You will carry out holistic OT assessments for individuals experiencing homelessness, work within a multidisciplinary team to improve access to community services, and support discharge planning in line with D2A principles.

Qualifications

• Registered Occupational Therapist with HCPC registration (essential).
• Experience in homelessness or housing-related health issues preferred.
• Ability to work in multidisciplinary teams and in discharge planning processes.

Responsibilities

• Carry out holistic occupational therapy assessments and interventions for people experiencing or at risk of homelessness across Oxfordshire.
• Work within an MDT to help clients access mainstream health and social care services.
• Assess and support care needs within Step Service accommodation in line with D2A principles.
• Contribute to ongoing development of the OT service within OHHIT and wider service delivery.
• Provide guidance to staff across health, social care and third sector on occupational needs and therapeutic approaches.
