Oxford Health is offering a 6-month secondment for an Occupational Therapist to join our community Therapy Team in West Oxfordshire. You will manage a rehabilitation caseload, contribute to discharge planning and work closely with multidisciplinary colleagues to deliver high-quality care in the community.

Applicants must have HCPC registration and a relevant degree, with Band 5 NHS experience or social care exposure preferred.

Qualificações

• HCPC registration is essential.
• Physiotherapy or Occupational Therapy degree (or equivalent) is required.
• Band 5 NHS experience or working in social services is desirable.

Responsabilidades

• Deliver safe and effective rehabilitative patient care and discharge planning in partnership with multidisciplinary colleagues.
• Assess, plan, implement and evaluate individualized care programmes using clinical reasoning and evidence-based practice.
• Work independently and collaboratively to manage a caseload and monitor patient progress.
• Communicate effectively with patients, families and carers, involving them in care planning and decisions.
• Develop and coordinate comprehensive discharge plans with health and social care professionals.
• Maintain accurate clinical records, manage risks and adhere to safeguarding and infection prevention requirements.
• Support service improvement, performance targets and the development of colleagues through mentoring and education.
