Job summary

Sanctuary Personnel is seeking a dedicated Family Support Worker to join the Children with Disabilities Team in Ilford. This permanent, full-time role involves supporting families, coordinating with health, education and social services, and developing care plans to improve outcomes for children with disabilities.

Qualifications

• Experience in supporting families of children with disabilities.
• Ability to assess needs and prepare care plans.
• Strong communication and collaboration with partners.

Responsibilities

• Undertake and monitor reviews of children with disabilities, working to minimise the effects of their disability.
• Facilitate access to appropriate and beneficial services for families and children.
• Identify and refer child protection issues to Senior Practitioners or the Team Manager.
• Develop strong links with Health, Education, Connexions, and Adult and Children’s Social Services.
• Complete various assessments and prepare reports to support children’s well-being and care plans.
• Work collaboratively with parents and professionals in planning and delivering services.
• Maintain and update comprehensive records using specified electronic systems.
