Job summary

You will conduct reviews, collaborate with multi-agency professionals, and contribute to care planning. The role may require flexible hours to meet diverse family needs and participation in team development.

Qualifications

• Experience reviewing children with disabilities and minimising impact.
• Facilitate access to services for families and children.
• Identify and refer child protection issues to Senior Practitioners or the Team Manager.
• Develop links with Health, Education, Connexions and Social Services.
• Complete assessments and prepare reports to support children's wellbeing.

Responsibilities
