Job summary

Southampton City Council invites applications for a Family Practitioner within the Family Help Team, West Locality. You will be based in one of the Family Hubs, delivering early help services to children, families and carers with a focus on assessment, planning and co-ordination of resources.

You'll hold Level 4 qualifications (BTEC, NVQ or equivalent) in Education, Health, Social Care or Early Years, or be working towards one.

Qualifications

  • Level 4 qualification in Education/Health/Social Care or Early Years, or working towards it.
  • Commitment to work towards a recognised qualification.
  • Experience in delivering early help and safeguarding.
  • Excellent collaboration with multi-agency partners.

Responsibilities

  • Deliver early help interventions and carry out early help assessments and planning (TAF).
  • Coordinate actions and resources to support children in need.
  • Provide advice and information to families to meet needs in partnership with carers/parents.
  • Safeguard and empower children, young people and vulnerable adults.
  • Participate in supervision and training to maintain professional standards.
  • Ensure accurate written records in line with Data Protection policies.
