Gloucestershire County Council is seeking an EHCP Case Co-ordinator to join the Education, Health and Care Plan (EHCP) Service in Gloucestershire. You will prepare, review and amend plans, liaising with families and professionals to ensure timeliness and accuracy.

Under supervision of an EHCP Lead, you will attend annual reviews, analyse professional advice, coordinate meetings and prepare information for panels, while maintaining case records and safeguarding at the core of practice.

Qualifications

• Five GCSEs or equivalent.
• Level 4 professional qualification or equivalent experience.
• Knowledge of SEND Code of Practice and person-centred planning.
• Ability to travel across Gloucestershire with a driving licence.

Responsabilités

• Prepare, review and amend Education, Health and Care Plans within statutory timescales.
• Attend annual reviews and coordinate meetings with families and professionals.
• Analyse professional advice and assessment reports to inform panels.
• Maintain accurate case records and provide clear guidance.
• Support transition planning and safeguarding principles in practice.
