Job summary

Coventry City Council in Coventry is recruiting an Education, Health and Care Plan Coordinator to join the STAR team. You will manage a caseload of pupils, support schools through the EHCP assessment process and attend annual reviews with a person-centred approach.

You will produce high-quality EHCPs for children and young people aged 0-25, coordinate bespoke educational programmes and monitor personal budgets to ensure value for money. DBS check applies.

Qualifications

• Experience in an inclusive role supporting children with additional needs.
• Knowledge of the 2014 SEND Reforms and the educational landscape.
• Ability to manage a caseload and coordinate multi-agency reviews.

Responsibilities

• Manage a caseload of pupils, supporting schools through the EHCP assessment process.
• Attend annual reviews and transition reviews with a person-centred approach and active parental/carer involvement.
• Produce high-quality EHCPs for children and young people aged 0-25, ensuring compliance with legislation.
• Coordinate and monitor bespoke educational programmes for young people on alternative provision.
• Monitor cases funded through personal budgets, reviewing outcomes and value for money.
