Job summary

HCRG Care Group in Swindon is seeking a Band 3 Discharge and Flow Coordinator to help patients go home safely by keeping wards moving. You will work with nurses, therapists and social care colleagues to coordinate discharge planning at Swindon Intermediate Care Centre and across BSW Community Hospitals.

The role is permanent full-time (37.5 hours per week) with a salary of £25,500–£27,098 FTE, access to a pension, wellbeing and learning opportunities, and a supportive, inclusive team culture.

Qualifications

• NVQ Level 3 in Care, Business Administration or equivalent experience.
• IT skills including Microsoft Office, Excel, Word, Email and MS Teams.
• Experience in health or social care and multi-disciplinary teams.
• Able to communicate professionally and empathetically with patients and families.

Responsibilities

• Support safe and timely patient discharges by completing all administrative tasks accurately.
• Visit patients on wards to update families on discharge plans.
• Explain discharge processes and expected timeframes to patients and carers.
• Attend MDT meetings and record actions and discharge readiness on clinical systems.
• Maintain and update electronic records and referrals for smooth discharge.
