Job summary

Mersey West Lancashire NHS Trust is seeking a Patient Discharge Facilitator to support the Patient Flow Team. The role requires collaboration with clinicians, social care, and multidisciplinary colleagues to ensure safe and timely patient discharges from Southport and Ormskirk Hospitals.

This 30-hour-per-week post (4 days) includes some Saturday work every 6-8 weeks; the postholder will utilise IT systems, communicate clearly, and manage discharge information and documentation to maintain smooth

Qualifications

• NVQ 4+ in Health/Social care or equivalent.
• Experience in acute or community settings.
• Knowledge of discharge planning processes.

Responsibilities

• Work closely with clinicians, social work teams and multidisciplinary teams to deliver a high quality patient focused service.
• Liaise with multi-disciplinary teams to facilitate accurate assessments of patient’s needs.
• Provide patients, relatives and carers with information about discharge planning and ensure timescales are understood.
• Liaise with pharmacy to ensure TTOs are ready for the patient’s discharge.
• Liaise with ambulance control when ambulance transport is required for the patient’s discharge.
• Liaise with care/nursing homes to ensure timely assessments and completion of required documentation to facilitate discharge.
• Attend discharge planning meetings and raise issues with the MDT to ensure resolution prior to discharge.
• Ensure discharge information is recorded accurately, processed on time and communicated to the MDT.
• Ensure awareness of hospital discharge policies is communicated to all relevant parties.
