Since October 2020 the responsibility for the commissioning of Adult Secure Care mental health, learning disability and autism services has transferred from NHS England (NHSE) Specialised Commissioning to Provider Collaboratives.
Fifteen Provider Collaboratives nationally coordinate and deliver care for adult patients requiring low and medium secure care.
The East Midlands area Provider Collaborative is called IMPACT.
IMPACT is a partnership of 9 NHS and independent sector providers and is led by Nottinghamshire Healthcare NHS FT as the Lead Provider.

In order to preserve integrity and avoid conflicts of interest, Nottinghamshire Healthcare Foundation Trust as the Lead Provider developed an arms-length commissioning and clinical oversight function (the IMPACT commissioning hub). IMPACTs commissioning hubs role involves understanding the local population and empowering local clinicians and Experts by Experience to design improved pathways of care. Through the Lead Provider arrangements IMPACT also sub-contracts other providers e.g. to support discharges from secure care, assure the quality of services and lead the necessary reporting regionally and nationally to NHSE.
The Senior Case Manager (Operational Lead) has a central role in the IMPACT hub’s Senior Leadership Team. The postholder will work in close contact with IMPACT’s Clinical Director, Managing Director and Lead Managers for Improvement, Contracting, Finance, Quality and Engagement.
The postholder will lead and line manage a team of case managers. As a team they provide credible oversight and facilitation of care for patients and have influence over commissioning plans and decisions. More specifically they support IMPACT’s commissioning hub and the wider provider collaborative.

Operational
Provision of clinical expertise to the team and wider PC alongside the clinical advisor.
Resolution of complex or disrupted care pathways in conjunction with colleagues across England (and within UK for patients placed in services outside England)
Support the Clinical Director in managing escalation in problems with access, pathway, transitions or discharges to services for service users in secure and non-secure services.
Ensuring case management processes and services within the PC are compliant to all specifications and guidance.
Ensuring national case management systems and record keeping are accurately maintained and kept up to date.
Work internally and externally to deliver projects, initiatives and services to deadlines and in a cost-effective way.
Operating with due regard to adult secure care being an area of work that can be a highly political and sensitive environment, with areas of confidentiality observed.
Work with ICS partners and their commissioned services within the East Midlands region to ensure a coordinated approach to care and discharge planning, which results in robust and effective care packages.
Managing individual patient pathways to ensure high quality services and care planning outcomes are achieved.
Take action in respect of Safeguarding Policies and Procedures as required.
Support the IMPACT portfolio of programmes/projects in demonstrating value for money tracking, managing and delivering agreed benefits.
Provide advice and prepare strategic reports and briefings for senior leads and stakeholders.
Provide in depth analysis, interpretation and production of complex and multiple reports.
People Management
Manage a team of case managers. This will include responsibility for supporting appraisals, development of staff, recruitment, managerial supervision and where necessary HR processes such as grievance and disciplinary matters.
Work in conjunction with other managers to assess and manage confidential information about an individual’s performance and capability development.
To support, motivate and develop people’s skills within the case management team.

Manage third parties (such as consultants/ interims) to ensure deliverables are met in a timely manner and within budget.
Pro-actively manage stakeholders, respond to and resolve conflict when this arises, through facilitation or other appropriate mechanisms.
Information Management
Ensure accurate and up to date record keeping and appropriate use of relevant case management systems (i.e. National Case Management System - NCMS) with due regard to Data Protection and Caldicott Principles.
Develop and deliver internal information sharing of intelligence and trends.
Conduct interpretation and presentation of complex data (qualitative and quantitative) from a range of information sources related to the role.
Provide reports summarising status on issues, appraising outcomes and providing progress reports as required.
Highlight exceptions and risks ensuring mitigating action is taken.
Drafting reports summarising status on issues, appraising outcomes, and providing progress reports as required for IMPACT senior leadership team.
Lead appropriate analysis to develop robust business cases and contribute to delivery of ‘products’ and service improvement.
Policy and Service Development
To be responsible for project work and service redesign to make improvements in clinical pathways.
Carry responsibility for developing policy and procedure within own discipline/field, including briefings, commissioning, business or resources.
Responsible for proposing and drafting changes, implementation and interpretation to policies, guidelines and service level agreements.
To be responsible for specific project work and service redesign to make improvements in the pathways for adult secure care.
To carry responsibility for developing policies and procedures in the area the jobholder is currently working on, including briefings, commissioning, business or resources.
Responsible for proposing and drafting changes, implementation and interpretation to case management policies and guidelines.
To lead the case management function’s contribution to the strategic development and implementation of the Transforming Care agenda, implementing local and national policy as required.
To lead the case management function’s interaction with ICS partners on service redesign and pathway improvements for patients.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.
