Job summary

Royal Borough of Windsor and Maidenhead (RBWM) is offering a full-time Social Care Practitioner role in Maidenhead. The post is hybrid with a salary of £26,895–£30,132 per annum and 37 hours per week.

The LAP team supports residents through multidisciplinary care, with priorities on Care Act assessments, caseload management, and providing guidance to residents and carers. A UK driving licence and IT proficiency are required.

This is a full-time, permanent role with hybrid working, and a salary of £26,895 - £30,132 per annum.

The role:

The Social Care Practitioner role is within the Integrated care team (LAP) which consists of Community Matrons, Mental Health Practitioners, Social Workers and Occupational Therapists.

The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way helping to prevent longer term challenges and admissions to hospital.

Your role will involve:

• Completing Care Act Assessments with residents and their carers.
• Holding and managing a case load.
• Assisting the duty function of the team, and providing advice and signposting to residents.

What we are looking for:

• An individual with a background or experience in working with people in a care setting/or similar.
• A good understanding of IT with skills including MS Office, Word, and Excel.
• Must hold a UK driving licence.
