Job summary

The Royal Borough of Windsor and Maidenhead is seeking a Social Care Practitioner to join our Locality Access Team on a full-time, hybrid basis. The role involves completing Care Act assessments, managing a caseload, and providing advice and signposting to residents to support independent living.

Qualifications

• Experience of working with people, preferably with vulnerable or disabled individuals.
• Current UK professional registration is required.
• This post requires compliance with the Rehabilitation of Offenders Act (Exceptions Order) 1975 and DBS checks.

Responsibilities

• To manage the allocated workload of residents and work with families and carers to achieve positive outcomes.
• To undertake Care Act assessments and reviews.
• To facilitate and deliver support interventions to meet identified outcomes and independence.
• To maximise engagement while ensuring safeguarding and risk management.
• To maintain up-to-date case records and evidence how services meet legal duties.
• To attend multi-disciplinary meetings and contribute to performance targets.
