Job summary

Surrey and Borders Partnership NHS Foundation Trust is recruiting a Community Perinatal Mental Health Practitioner to cover the Southwest team at Berkeley House, Godalming. You will assess, plan, implement and evaluate care for women accessing the service, mentoring staff and supervising students within a multidisciplinary team.

Qualifications

• Must hold a professional qualification in health or social care and be registered with the appropriate body.
• Experience in perinatal mental health is advantageous.
• Demonstrated commitment to ongoing professional development.

Responsibilities

• Be accountable for a defined caseload and assess care needs under supervision.
• Ensure effective referral management, triage, and timely assessments across the service.
• Respond to emergency referrals and liaise with HTT, inpatient services and MBU.
• Participate in multidisciplinary assessment and provide perinatal mental health nursing expertise.
