Job summary

NFP People is recruiting a Social Prescribing Link Worker to support patients in Birmingham & North Solihull, based in GP surgeries. The role is part-time (up to 25 hours) with hybrid working where possible.

You will join a PCN multi-disciplinary team to link individuals to community resources and co-create simple personalised care plans to improve health and wellbeing. The ideal candidate has a Level 3 qualification in Health & Social Care (or working towards) and a strong commitment to

Qualifications

• Level 3 qualification in Health & Social Care or related field, or working towards it.
• Commitment to professional and personal development.
• Experience in community-based contexts and supporting people with mental health needs.

Responsibilities

• Take referrals from GP practices within the local PCN and provide personalised support.
• Develop trusting, holistic care plans and connect people to community resources.
• Collaborate with local partners and educate staff about social prescribing benefits.
• Promote awareness of services and map local assets to reduce health inequalities.
