Job description

We are seeking a motivated and compassionate Social Prescribing Link Worker to join our team, supporting individuals to improve their health, wellbeing, and independence through personalised, non-medical interventions and community-based support. The successful candidate will work closely with GP practices, healthcare professionals, voluntary organisations, and community groups to help people take greater control of their health and wellbeing. Using a person-centred approach, you will identify what matters most to individuals and connect them with local services, activities, and support networks. You will support people facing social, emotional, and practical challenges, including loneliness, poor mental wellbeing, financial difficulties, housing concerns, and long-term health conditions. By helping individuals access appropriate support, you will contribute to improved wellbeing and reduced health inequalities. This role is ideal for an organised, proactive, and empathetic individual with strong communication and relationship-building skills. You will work across GP surgeries, community venues, and patients' homes, supporting people to engage with services that enhance their wellbeing and quality of life. Please note: Interviews for this role will be taking place on 3rd and 5th of November. This role requires the successful candidate to hold a full driving licence and have access to a vehicle. Applicants who do not meet this essential requirement should not apply.

  • Knowledge of the personalized care approach and providing personalized support
  • Work alongside individuals to identify goals and formulate simple care plans to support their wellbeing.
  • Meet people on a one-to-one basis, making home visits where appropriate within organizations policies and procedures. Give people time to tell their stories and focus on what matters to me. Build trust and respect with the person, providing non-judgmental and non-discriminatory support, respecting diversity and lifestyle choices. Work from a strength-based approach focusing on a persons assets.
  • Help people identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities.
  • Work with the person, their families and carers and consider how they can all be supported through social prescribing.
  • Help people maintain or regain independence through living skills, adaptations, enablement approaches and simple safeguards.
