Job summary

Not For Profit People is recruiting a Social Prescribing Link Worker to support primary care networks in Birmingham and North Solihull. The role focuses on holistic health, connecting people to community resources and emotional support, with a strong community-first approach.

The post is part-time up to 25 hours per week, offering hybrid working, flexible hours and family-friendly policies. Closing date is 25 October 2026 with rolling interviews.

Qualifications

• Level 3 qualification in a relevant field (Health & Social Care, Health Trainer, Health Improvement, Community Family Worker, Psychology) or working towards.

Responsibilities

• Take referrals from GP practices within the local PCN.
• Provide personalised support to individuals, families and carers to improve health outcomes.
• Develop holistic care plans and connect people to community groups and services.
• Collaborate with local partners to map assets and share intelligence with commissioners.
