Job summary

Rochdale Council is seeking a social worker to join the Preparing for Adulthood, Learning Disability and Autism Team. The role focuses on supporting adults with complex needs, families and carers to live independently using a strength-based approach.

You will collaborate with integrated neighbourhood teams and Adult Mental Health Services to deliver outcome-focused care. Appointment is subject to DBS and background checks. Hybrid working is offered.

Qualifications

• Experience working with individuals with Learning Disabilities and/or Autism.
• Care Management experience in the Adult Care sector.

Responsibilities

• Support service users, families and carers to live as independent as possible through a strength-based approach.
• Work closely with integrated neighbourhood teams, front door team and Adult Mental Health Services to deliver outcome-focused services.
