Job summary

South London and Maudsley NHS Foundation Trust is seeking an Early Years High Needs Co-ordinator to support families across Southwark. The role blends home visits with office work, focusing on children aged 0-5,SEND pathways and multi-agency collaboration.

You will assess development, produce reports, and help families navigate EHCP processes while promoting inclusive practice across services. Hybrid working with travel across Southwark.

Qualifications

• Level 3 in Early Years or SEND qualification.
• Understanding EYFS, SEND Code of Practice (0-25), Portage, safeguarding.
• Experience with disabilities/additional needs/SEND.
• Experience of home-based outreach and multi-agency work.
• Strong report writing and IT skills.
• Ability to manage a caseload independently.

Responsibilities

• Undertake home visits and developmental assessments for children aged 0-5.
• Coordinate interventions and triage health notifications.
• Build positive relationships with families.
• Deliver play-based interventions to support development.
• Support SEND pathways, referrals and EHCP processes.
• Coordinate multi-agency support plans across Early Years, Health, Family Hubs, Family First and voluntary sector partners.
• Produce professional reports and recommendations.
• Promote inclusive practice and access to local services.
• Support transitions into education settings.
• Ability to travel across Southwark locations.
