Job summary

Qualifications

• Full Level 2 or 3 qualification in adult social care (e.g., Diploma in Care).
• Experience with adults in a social care setting and inter-agency work.

Responsibilities

• Undertake Care Act assessments and develop outcome-based support plans.
• Identify and manage risk, safeguarding, and mental capacity issues.
• Work in a hybrid role with visits to homes, hospitals and care settings.
