Job summary

Leicestershire County Council is recruiting a Community Support Worker for the Cognitive and Physical Disability Team in North West Leicestershire. The role involves direct work with adults, families and carers, conducting assessments and creating support plans to promote independence and wellbeing.

You will identify risks, carry out Mental Capacity Assessments and Best Interests decisions, and work across settings, including homes, hospitals and care facilities.

Qualifications

  • Hold a Level 2 in Maths and English and relevant health/social care qualifications, a full Level 2 or 3 qualification in adult social care (e.g. NCFE CACHE Level 2 Diploma in Care).
  • Experience of working with adults in a social care setting and collaboration with other agencies.

Community Support Worker (Career Grade) – Cognitive and Physical Disability, North West Leicestershire

Salary:£29,550 - £35,493 per annum (pro rata for part-time)

About the Role

As a Community Support Worker, you will work directly with adults, their families and carers, building positive relationships through meaningful conversations that help you understand what matters most to them.

You will undertake Care Act 2014 assessments, identifying strengths, personal outcomes and areas where support may be needed. Where eligible needs are identified, you will work with the person to develop a support plan that promotes independence, wellbeing and choice. This may include connecting people with community resources, arranging statutory services, or referring to partner agencies and support networks.

You will also play an important role in identifying and managing risk. This includes recognising safeguarding concerns, undertaking Mental Capacity Assessments and Best Interests decision making, and responding appropriately where a person's safety, wellbeing or liberty may be at risk.

This is a hybrid role, with a combination of office based working, home working and visits within the community. You will meet people in a variety of settings including their own homes, hospitals, care homes and other locations where they feel comfortable and able to participate in conversations about their care and support needs.

About the Team

The Cognitive and Physical Disabilities (C&PD) Team supports adults with physical disabilities and older people with eligible care and support needs arising from frailty, cognitive impairment and long-term health conditions. This includes, but is not limited to:

  • Dementia
  • Stroke
  • Parkinson's disease
  • Acquired brain injury
  • Other long-term neurological and physical health conditions

Our focus is on promoting independence, maximising strengths and supporting people to achieve outcomes that improve their wellbeing and quality of life.

You will work as part of a locality-based multidisciplinary team alongside Social Workers, Community Support Workers and other professionals. You will be expected to work collaboratively with colleagues across partner organisations to support people to achieve their outcomes and maintain their independence.

As a Grade 8 Trainee Community Support Worker you will be in your first CSW role and will be expected to develop experience, knowledge and skills, complete their Learning and Induction Programme and undertake your duties with support and supervision.

As a Grade 9 Community Support Worker you will be expected to have sufficient experience, knowledge and skills to manage your workload under supervision, to deliver a high quality, person centred, accountable statutory social care. This includes demonstrating competency undertaking Care and Support Assessments, Mental Capacity Assessments and making Best Interest decisions.

About You
To apply for the CSW post, you must:
  • Hold a Level 2 in Maths and English and relevant health/social care qualifications, a full Level 2 or 3 qualification in adult social care, for example NCFE CACHE Level 2 Diploma in Care, or equivalent.
  • Have relevant experience of working with adults in a social care setting and of working with other agencies within a fieldwork environment.
  • Understand the need to undertake need and risk assessments and the issues/needs that adults and their families/carers may have.
  • Have a good understanding of social care organisations and their roles and responsibilities as part of the wider community and voluntary sector.
  • Be a confident communicator who is skilled in negotiation, persuasion, assertiveness, and conflict management, and able to build effective working relationships.
  • Be able to recognise the limits of own accountability and responsibility, and to seek appropriate support and advice when required.
  • Be able to manage a caseload and work to clear objectives.

Additional criteria:

You will require a valid full driving licence or access to a vehicle for this position
