Qualifications

• Substantial experience of working and engaging with young people.
• Experience of working with a variety of agencies and social care professionals.
• Experience of dealing with sensitive and confidential matters, pressure and conflict.
• Ability to formulate plans for intervention and service provision for young people.
• Full driving licence and access to a vehicle daily.
• Enhanced DBS check.

Responsibilities

• Provide advice and practical support to care leavers on financial capability, accommodation, education, training, employment, leisure and culture.
• Participate in assessment, preparation, monitoring and review of Pathway Plans with a person-centred approach.
• Promote and facilitate contact for young people with their birth family members; provide supervision and assessment.
