Job summary

Staffordshire County Council's First Contact Team seeks a dedicated assessor to deliver Care Act compliant assessments by telephone and in-person across the county. This full-time hybrid role requires two days per week in the office, with onboarding in the office.

Weekend work may be required in the future based on demand. You will build collaborative relationships with adults, carers and professionals, analyse information to determine needs and outcomes, and help individuals achieve

Qualifications

• Care Act knowledge and policy context for adult services.
• Understanding disability, carers and their impact on individuals and families.
• Knowledge of the Mental Capacity Act and its application.
• Experience using IT to create documents, records, diaries and reports.
• Ability to build collaborative relationships with adults with disabilities and their families.
• Ability to communicate clearly to determine needs and enable informed choices.

Responsibilities

• Take responsibility for provision of Care Act assessments including statutory duties.
• Develop collaborative relationships with adults and their circles of support.
• Undertake purposeful interactions to understand needs and capacity to consent.
• Analyze information to identify risks and develop mitigation strategies.
• Negotiate outcomes with individuals and families.
• Collaborate with professionals to understand needs and promote independence.
• Maintain professional records in line with policies and data standards.
• Inform practice using research findings and case discussions with peers.
• Contribute to shaping and improving Adult Social Care services.
• Participate in supervision, performance data reviews and CPD.
• Coordinate case reviews with social workers and other specialists, inviting involvement when needed.
• Exercise flexible working in response to demand and cover weekend duties as required.
