Job summary

Norfolk County Council is seeking a Supported Assistant Practitioner to work in Adult Social Services. The role involves conducting face-to-face visits, assessments and reviews within local communities, and supporting individuals to live independently with person-centred care.

The post is a permanent contract, 37 hours per week, with a hybrid working pattern across community settings and office days. Salary ranges (£30,023–£32,046 F Grade or £33,119–£34,811 G Grade) and excellent staff benefits

Qualifications

• You will have experience in the Health or Social Care sector.
• You will have a good standard of education and excellent IT/organisational skills.
• You will be able to work independently and as part of a wider team.

Responsibilities

• Carry out face-to-face visits, assessments and reviews in homes and community settings.
• Work in a hybrid model with two days per week in an office or base location.
• Support adults to remain as independent as possible within their local communities.
