Job summary

Norfolk County Council is seeking a Supported Assistant Practitioner/Assistant Practitioner in King's Lynn for a hybrid role across local communities. You will visit clients at home or in community settings, conduct assessments and reviews, and work within a team to promote wellbeing and independence.

A strong IT and interpersonal skillset is required to support people effectively. The role offers progression via a development programme, with salary bands from £30,023 to £34,811 per annum

Qualifications

• Experience in Health or Social Care sector
• Good standard of education
• Ability to use IT effectively and maintain records

• Conduct face-to-face visits, assessments and reviews in people's homes and community settings
• Contribute to service delivery and team development
