Job summary

Change Grow Live in Eastbourne is seeking a Housing Support Recovery Worker to assess and engage service users, helping them access the right treatment and care options. You will work with Housing Officers to address housing needs and promote tenancy sustainment.

You will carry out screenings, assessments and care planning, deliver harm minimisation training, and coordinate treatment across outreach sites. A full UK driving licence and use of your own car are required.

Qualifications

  • Minimum of 12 months experience facilitating successful tenancy sustainment.
  • Experience engaging with individuals from diverse backgrounds and needs.
  • Ability to work with and manage challenging behaviour.

Responsibilities

  • Establish referral pathways for individuals at risk of homelessness.
  • Provide wrap-around support in emergency accommodations across the area.
  • Prioritise caseload based on crisis intervention and severity of needs.
  • Attend multi-disciplinary meetings linking health and social care services.
  • Work with local authorities and housing partners to prevent homelessness.
  • Support budgeting, education, training and employment activities.

Overview

Change Grow Live are a charity dedicated to the belief that we can make a difference to our Service Users lives, offering support and respect in a safe environment, treating each user as an individual and working with them to find the right treatment and care options. We have an exciting opportunity for a Housing Support Recovery Worker who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs. The role will also involve engaging services users and supporting them to navigate treatment, working in partnership with Housing Officers to assist with addressing substance misuse issues by promoting engagement and whilst working with Housing Officers to identify those clients that exist within our services who need support with tenancy sustainment. The post-holder will be part of a team that provides screening, comprehensive assessments, and care planning for new service users in outreach, delivering harm minimisation training and interventions as well as coordinating treatment for clients identified as having a housing need.You must hold a full UK driving licence and business insurance to apply for this roleWhere: EastbourneFull- Time Hours: 37.5 per weekContract Type: 12 Month Fixed Term Contract

Full-time hours at Change Grow Live are 37.5 hours per week. For part‑time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

  • Establish effective communication and referral pathways for individuals who are at risk of homelessness and in contact with other health and care agencies.
  • Provide wrap around support in emergency/temporary/supported accommodations across the area.
  • Prioritise caseload depending on crisis intervention and severity of needs.
  • Attend relevant multi-disciplinary team meetings supporting links to health and social care services, peer support networks, community assets and mutual aid.
  • Work closely with the local authority, Health & Social care sector, partners and any provision linked to preventing Homelessness and improve joint working.
  • Increase and enhance partnership working with primary care, mental health services, housing and other voluntary organisations who are supporting individuals at risk of losing tenancies.
  • Provide wrap around support on budgeting, education, training and employment whilst continuing to support their engagement in structured services.

About you:

  • An understanding and knowledge of the harmful effects associated with drug and alcohol use in relation to health, social welfare, housing, employment and personal relationships.
  • Minimum of 12 months experience facilitating successful tenancy sustainment and working to achieve positive community connections.
  • Experience of engaging with a wide variety of individuals with a variety of backgrounds and needs.
  • The ability to work with and manage challenging behaviour.
  • Have a valid UK driving licence, and use of own car, to work flexibly across different sites as required.
