Job summary

Action for Children is seeking a Housing Support Practitioner in Dundee to provide tenancy and outreach support for vulnerable young people aged 16-21 who are homeless or at risk of homelessness. The role focuses on housing advice, conflict resolution, school-based education, group work and one-to-one support to help young people build resilience and life skills.

You will work within the Dundee Whole Family Service (Homeless Prevention Team) and collaborate with partner agencies to deliver

Qualifications

  • SVQ level 3 in Health & Social care or equivalent.
  • Experience in multi-agency social care, early years or health environment and applying safeguarding procedures.
  • Experience of managing cases, completing assessments, planning and reviews to ensure positive outcomes.
  • Knowledge of the issues and barriers facing young adults who may at times lead chaotic lives.
  • Knowledge of disabilities and how this affects individuals.

Responsibilities

  • Offer general counselling and support including befriending, advising on food preparation, independent living skills, housing applications and tenancy sustainment.
  • Signpost young people to services that are relevant to their needs.
  • Maintain effective working relationships and communication with colleagues, young people and families.
  • Be alert to child protection and safeguarding policies and procedures.
  • Participate in regular supervision sessions and practice meetings to maintain a cohesive approach.
  • Ensure familiarity and compliance with relevant policies and procedures.
  • Work towards GIRFEC outcomes and comply with Health and Social Care Standards and the SSSC Code of conduct.
